Unexplained Infertility Isn't Always Unexplained: 3 Root Causes That Often Go Unexplored

My hope is that this blog can be both a resource and a reminder that you're not alone.

Is Unexplained Infertility Really Unexplained?

The conventional approach to fertility often follows a fairly predictable path:

  • Try to conceive for a year.

  • If pregnancy doesn't happen, schedule an evaluation.

  • Consider IUI.

  • If that isn't successful, move on to IVF.

For many couples, these interventions can absolutely have a place.

But what often gets overlooked is a deeper question:

Why isn't pregnancy happening in the first place?

Instead of simply asking what intervention should we try next?, I believe we should also be asking:

What's happening beneath the surface that's affecting fertility for you and your partner?

Because fertility struggles are rarely unexplained.

They're often unexplored.

Fertility is about so much more than ovulation strips, timed intercourse, and reproductive technology.

Your body has to feel safe, nourished, and supported enough to prioritize reproduction.

Here are three root causes that frequently impact fertility but aren't always thoroughly investigated during a conventional fertility workup.

1. Gut Health, Infections & Immune Activation

Your gut isn't just responsible for digestion.

It's also home to roughly 70% of your immune system, making it one of the biggest regulators of inflammation and immune function throughout the body.

When the gut is inflamed or imbalanced, it can contribute to chronic immune activation—and that matters for fertility.

An embryo is technically recognized as genetically "foreign" to the body. A healthy immune system doesn't attack it, but an immune system that's chronically activated or dysregulated may have a harder time supporting implantation and early pregnancy the way it's designed to.

One infection I commonly see in practice is Helicobacter pylori (H. pylori)—a bacteria estimated to affect roughly half of the world's population.

Research has linked H. pylori to fertility challenges, and studies have even identified H. pylori antibodies in cervical mucus, where they may interfere with sperm's ability to reach the egg.

Yet gut health is often only minimally evaluated—or not evaluated at all—as part of a traditional fertility workup.

2. Poor Cellular Energy & Inadequate Fuel

Whether it's chronic dieting, under-eating, excessive exercise, insulin resistance, or simply not meeting your body's nutritional needs, fertility requires an incredible amount of energy.

Egg cells contain approximately 800,000 mitochondria—your body's tiny energy factories.

For comparison?

Heart muscle cells contain only about 7,000.

That's because growing healthy eggs, producing hormones, supporting implantation, and sustaining pregnancy are incredibly energy-intensive processes.

When your body doesn't have enough fuel, it prioritizes survival before reproduction.

Sometimes fertility isn't simply about hormone levels.

Sometimes it's about whether your cells have enough energy to do what they're being asked to do.

3. Endocrine-Disrupting Chemicals (EDCs)

Endocrine-disrupting chemicals are substances that can interfere with hormone signaling throughout the body.

They're commonly found in:

  • Plastics

  • Pesticides

  • Personal care products

  • Fragrances

  • Household products

Unfortunately, they're difficult to avoid completely.

Research has linked certain endocrine disruptors to:

  • Reduced ovarian function

  • Poor sperm quality

  • Ovulatory dysfunction

  • PCOS

  • Endometriosis

  • Fibroids

  • Adverse pregnancy outcomes

While reducing exposure won't solve every fertility challenge, it's one piece of the puzzle that's often overlooked when trying to optimize reproductive health.

Looking Beyond Basic Fertility Testing

These three areas—gut health, cellular metabolism, and environmental exposures—are often missing from a conventional fertility evaluation.

Most fertility clinics rely primarily on standard fertility labs, which provide valuable information but only tell part of the story.

In my practice, I take a more comprehensive, root-cause approach by using functional testing alongside a detailed review of your health history, nutrition, symptoms, and lifestyle.

Depending on your situation, that may include:

  • Comprehensive stool testing to evaluate gut health, infections, inflammation, and immune activity

  • Hair Tissue Mineral Analysis (HTMA) to assess mineral status and cellular metabolic patterns

  • Comprehensive blood work to evaluate inflammation, immune markers, thyroid function, nutrient status, and other factors that may influence fertility

We use those results to create a personalized plan that supports your body as a whole—not just your ovaries.

Nothing in fertility is guaranteed.
